天博体育 is proud to announce the opening of the fiscal year 2025 (FY25) Community Benefit Grant application period.
FY25 grant applications opened December 13, 2023. The deadline to submit applications is February 23, 2024, at 5 p.m. (PST). Award notifications will be sent in late June 2024, for funds to be used from July 1, 2024, through June 30, 2025.
Our Community Benefit Grant Program has long provided financial support for safety net health clinics, school district health programs, community service agencies and nonprofit organizations that support community health.
天博体育 uses information from the most recent 2022 Community Health Needs Assessment (CHNA) to inform the Community Benefit grant-making process, which is described in 天博体育's annual Implementation Strategy Report and Community Benefit Plan. The CHNA is conducted every three years by 天博体育 in collaboration with other local health systems.
The five health needs selected are:
- Healthcare Access and Delivery (including oral health)
- Behavioral Health (including domestic violence and trauma)
- Diabetes and Obesity
- Chronic Conditions (other than diabetes and obesity)
- Economic Stability (including food insecurity, housing and homelessness)
